Proxy Form Instructions

To participate in the upcoming Epperson Ranch II Community Development District election, you can assign a proxy to vote on your behalf if you cannot attend the meeting.

How to Fill Out the Proxy Form:

1. Download the proxy form and print it, or request a physical copy.

2. Fill in the following information on the second page:

  • Proxy holder appointee is Michele Frank

    text surrounding a blank line followed by the words Proxy Holder in quotation marks within parenthenses
  • Your name (printed and signed)
  • Your property's address, legal description, or tax parcel ID
  • The number of votes you are authorizing

3. Ensure the form is signed by one legal property owner.

Where to Submit the Proxy:

You can bring the completed proxy form to Michele Frank, or drop it off at the dropbox located at 8060 Abby Brooks Cir, Wesley Chapel South, FL 33545.
